We are looking for experienced Advanced Practitioners to join our clients Clinical Assessment Service in York. This is a dynamic, fast-paced, and patient-centred position where you will deliver high-quality urgent care assessments and make clinical decisions via telephone consultations. This is not a remote role and all shifts must be conducted from a base site in York.
Key Responsibilities
- Conduct comprehensive clinical assessments and effectively triage patients
- Diagnose and manage both acute and chronic conditions
- Prescribe appropriate medications and treatments within professional scope of practice
- Request, interpret, and act upon diagnostic tests and results
- Refer patients to relevant services or specialists as required
- Promote preventative health measures and provide patient education
- Manage long-term conditions such as asthma, diabetes, and hypertension
- Maintain accurate, timely documentation using SystmOne
Essential Requirements
- Current NMC or HCPC registration
- Independent Prescriber qualification
- Minor Illness qualification at a minimum of level 6
- Proven experience in urgent care, primary care, or walk-in centre settings
- Strong clinical decision-making and communication abilities
- Ability to work both independently and collaboratively within a multidisciplinary team
Shifts patterns are Monday-Sunday (08:00-13:00. 13:00-18:00 & 18:00-23:00). Shifts can be combined to make longer shifts.
