We are providing urgent support to a Rehab and Recovery Unit and require a part time ACP, up to 25 hours per week.
The ACP would work alongside the Lead Consultant and Nursing team to support with patient reviews, clinical interventions, medical summaries, medication management and prescribing.
Sub-acute care is defined as:
“Specialised multidisciplinary care in which the primary need for care is optimisation of the patient's functioning and quality of life”
The sub-acute model has been introduced to offer clinical pathways that can support post-acute care (or avoid hospital admission altogether). This can be achieved by either same day admission from Virtual ward for any sub-acute intervention or if appropriate working with SaTH Frailty Team to pull directly from ED. Sub-acute care within this setting can therefore be accessed by people across a range of areas, for example, pulmonary disease, cardiac disease, long term respiratory conditions, stroke/neuro rehabilitation, bariatric rehabilitation, cancer (chemotherapy), dialysis,
complex wound care and conditions requiring IV therapy or tube feedings.
