Pharmacist 8a

My client in Nottingham is seeking a Locum 8a Pharmacist for a 3-month assignment, working 20 hours per week. Please find the role details below:

Role Overview:

• Start date: As soon as possible
• Duration: 3 months
• Hours: 20 hours per week (must include either a full Tuesday or Tuesday afternoon)
• Location: Integrated Care Home Service

Key Responsibilities:

• Conduct medication reviews for new care home residents who have been assessed by a specialist nurse practitioner.
• Attend a weekly MDT meeting every Tuesday afternoon with a consultant geriatrician, specialist nurses, and other healthcare professionals to discuss patient care and ongoing medical management.
• Apply up-to-date pharmacological knowledge to support effective medicine management for conditions commonly seen in the older population.
