Triage Nurse

We are seeking an experienced Triage Nurse to join our Clients team and manage the care of 6,500 patients via phone. This is a temporary role for 6-8 weeks.

Location: Bracknell

Contract Duration: 6-8 weeks

Working Hours: Monday to Friday, 08:00 - 18:00

Salary: £21.97 per hour (PAYE)

Key Responsibilities:

• Patient Management: Handle triage for 6,500 patients using a traffic light system. Nurses will focus on "red" patients who require immediate attention.
• Questionnaire Review: Monitor weekly and daily patient questionnaires to assess their health status, ensuring timely and accurate reporting
• Status Monitoring: Utilize advanced monitoring software to track patient conditions and identify those in need of urgent care.
• Immediate Response: Contact "red" patients to provide immediate support and make necessary referrals, including to General Practitioners (GP), Urgent Community Response (UCR) teams, or emergency services as required.

Requirements:

• Registered Nurse with valid NMC registration
• Experience in triage or similar roles
• Excellent communication and organizational skills
• Ability to work independently and manage a large patient cohort
• Proficient in using monitoring and reporting software
